If you have been referred by your rehabilitation counselor, please read the following to help prepare you for your evaluation.

 

WHAT SHOULD I KNOW BEFORE THE VOCATIONAL EVALUATION?

 

Please arrange to fully participate in the evaluation, making no conflicting appointments on the days you have been scheduled. It is important to start on time at 9:00 a.m. and to stay until the session ends each day at 3:00 p.m. There is a scheduled one hour break at noon.

 

Please feel free to bring a lunch each day to the evaluation center. We have both a refrigerator and a microwave available for your use. If you prefer to eat out, there are many fast food and sit-down restaurants close by.

 

Children are not allowed in the evaluation center, so please make any childcare arrangements necessary. Please make certain to bring any physical assists that will make your stay at the evaluation center more comfortable (glasses, cane, cushions, braces, etc).

 

If you think you may have problems with attendance, please contact your rehabilitation counselor, or contact the FACTS evaluation center to which you have been referred.
